Case Study # 15
A 35-year old Native-American male presents in the clinic with a complaint of recurrent low-grade fevers, sweating, weakness, muscle pains and a loss of about 10% of his body weight over a 4 month period. The worsening weakness and muscle pain prompted the visit.
The patient reports that he has been working on a bison slaughter line owned by his tribe, and blood and tissue juices often splash in his face or contaminate minor hand and arm injuries. He likes to hunt and fish. He is married and has two children. He eats fairly well, but he has not been as hungry lately. He had rheumatic fever as a child and was in a fairly serious car accident three years earlier, resulting in a leg broken in three places. He feels he was physically fit prior to the past 4 months. He likes to play basketball and softball. He has had all the usual childhood immunizations, but does not see the doctor often.
On physical examination, the patient has a temperature of 101F. He has mildly swollen lymph nodes in the neck and under the arms. He has blood pressure of 136/86, and a normal heart rate without a heart murmur. There is no evidence of acute respiratory or gastrointestinal infection.

Open Question: I need help ASAP about my panic attacks?
Okay so I am having these panic attacks coming out of no where. I take adhd medication 60mg of focalin and 30-40mg of ritalin. I thought they were responsible for my panic attacks but instead they actually make it better! For example, this morning I took 3 focalin and I felt great and calm until 12pm hit over 6 hours! Medication only lasts 4 hours. I wanted to check my heart rate, and it said 100. I started freaking out, I felt dizzy then thoughts about a heart attack transpired then chest pain happened, I felt dizzy and I called 911. The ambulance came and the lady took my pulse and it was 80! She said I had a panic attack and there was no need for me to go to the hospital. The problem is these panic attacks are constant. I dont want to take medication at all becuase I already take ADHD medication, lamictal and wellbutrin. What can I do when I have these attacks to calm down. By the way I am super paranoid about my heart. When I walk I have to check my pulse, I check my heart all of the time, I take tons of omega oils because I fell I have heart disease and at one point was taking coq10 2000mg because I thought i had pcv. I got an ekg taking my focalin and the doctor said if I were to have a heart attack everyone would. I dont know, its like I dont trust them..uhhh I hate this. Ironically the only thing that takes away my panic attack is cigarrets and I am trying to quit. In fact when I stopped smoking this week I had tons and tons of panic attacks. help???
I never used to get them at all, until recently. I stopped running, is that the reason. I used to feel so relaxed after I ran, but I have no motivation at all after I added wellbutrin. But then I am deppressed with out it. I had a panic attack after I came from the city too many people and I was shaking and everything and people thought i was drunk or high because i was “out of it”. However, after a brisk walk home I felt so great! Should I do this when a panic attack happens!
