Resolved Question: Nursing diagnosis question?
Are nurses able to “diagnose” a heart murmur? For example, if you were listening to heart sounds (S1, S2) and heard a systolic murmur, would you simply chart that you heard an abnormal sound?
Would a medical diagnosis be “Heart Murmur due to Mitral Valve Prolapse” for example, and a nursing diagnosis be “Murmur heard” or “abnormal S1/S2 sound”?
Are nurses able to diagnose something like a heart murmur?
Would a nursing dx be hypoxia or cyatonic, whereas a medical dx would by hypoxia due to TB?
P.S. This is not related to an actual case, just curious.
Thanks!
Thanks. I’m in Canada too. A nursing diagnosis isn’t really a dx, but more a describing of symptoms or potential risks for a patient. I just want to know exactly *what* I would chart if I heard a murmur. Since I’m a pre-med *and* LPN student, I’m really having a hard time trying to look at how to “diagnose” from a nursing perspective. To me, a nursing diagnosis just seems “incomplete”, if that makes any sense. I keep looking at it from a medical dx perspective I think…
